NCLEX-RN or NCLEX-CAT has four choices and one multiple-choice question. The number of questions in the exam depends on the performance of the candidates. A minimum of 75 questions and a maximum of 265 questions. The test time is 5 hours, with a 10-minute break after 2 hours. During the test, the computer only displays one question at a time, and each question must be answered, otherwise, the next question will not be displayed. Questions that have already been answered cannot be viewed or modified. The computer will give subsequent exam questions based on the quality of the candidates' answers. If the candidate answers correctly, the computer will increase the difficulty of the test questions; if the candidate answers incorrectly, it will reduce the difficulty of the test questions. Test scores are determined by the difficulty of correct answers, not the number of correct answers. Therefore, when the computer makes a judgment on the examinee's level (that is, qualified or unqualified) based on the difficulty and accuracy of the examinee's answers, the examinee's exam will be stopped. If not sure, the examinee has to continue answering the questions until the computer can judge the examinee's level. The test is also over when 5 hours (including pre-test practice and rest between tests) or when candidates have completed 265 questions.


Currently, the passing standard of the 2003 American Nurse Licensing Examination is negative 0.28 (-0.28) for RN and negative 0.47 (-0.47) for LPN/LVN. That is to say, for an RN candidate, if the proportion of correct answers to the questions is always above the negative 0.28 line, it means that the quality of the candidate's answers is above the standard. Generally, when an examinee takes an exam, the computer has a question bank of 3,000 questions for the examinee to use. The computer can choose various difficult and easy questions for the examinee to answer. The computer calculation method of the American Nursing Licensing Examination does not require candidates to have any computer knowledge, and the only two keys used on the computer keyboard are Space Bar and Enter. Candidates use the Space Bar to move and the Enter Key to enter the correct answer. Now the computer mouse is added to make it more convenient to use. Before the official exam, there will be three sample questions to teach candidates how to use the computer. The formal exam questions will not appear until it is established that candidates know how to use a computer. During the exam, only one question is displayed on the computer screen at a time, and each question must be answered before the next question can be reached, otherwise the next question will not be displayed. Questions that have already been answered cannot be checked or modified.

The content of the NCLEX-RN (Registered Nursing License Examination) is formulated according to the knowledge and abilities that new graduates of American nursing schools should have. It is divided into two parts: nursing theory and clinical practice, including five traditional areas of nursing work, namely internal medicine, surgery, obstetrics, pediatrics, and psychiatry, take the overall comprehensive examination, not the sub-subject examinations.
